Job description

Voluntary Action Rotherham (VAR) is the lead body for supporting, developing and promoting the voluntary and community sector in the Rotherham Borough. Our role is to support and develop effective action to bring about positive change in the lives of individuals and communities in the local area.  We are looking to recruit further Link Workers (Social Prescribing) to join our team  to work within Rotherham’s Primary Care Networks (PCNs) to complement the existing social prescribing service.

As a Link Worker – Social Prescribing, you will be responsible for providing personalised support to individuals referred by the Primary Care Network (PCN), to help and empower them to improve their health, wellbeing and independence.  The successful candidates will work within a specific PCN in Rotherham and focus on specific areas of need and patient cohorts, working intensively with individuals to determine “what matters to them most”.

Forging strong links with local voluntary and community sector (VCS) organisations, community and neighbourhood level groups to utilise their networks and existing services, you will also be responsible for supporting local partners and commissioners to develop new groups and services where needed.  A key part of this role will be to encourage others to provide peer support and volunteering in order to build their skills and confidence, develop new support groups and strengthen community resilience.

As well as having experience of working within the VCS in a paid or voluntary capacity, the successful candidates must have an understanding of the wider determinants of health including social, economic and environmental factors.  As well as being a team player, you must have the ability to motivate and influence others through a person-centred approach and possess personal drive and resilience.

The successful candidates must be confidential, mindful of safeguarding issues and be willing to work flexible hours when required to meet work demands.  Being able to travel around the Borough is a key requirement of this role.

Please note during normal circumstances these roles will be based within GP Practices, however during COVID-19 restrictions you may be expected to work from home or Voluntary Action Rotherham’s office base.
