
Mental Health Community Connectors
A Mental Health Community Connector is there to provide one-to-one support to people living with Severe Mental Illness (SMI) across Rotherham. If you’re living with an SMI, we can support you to access your annual physical health check; build connections in your community; and help you to find the right service available for your needs.
This support may include:
Supporting to access funded physical health provisions
Connect to community groups and activities
Mental well-being support and resources
Support to access vaccinations and medication reviews
Employment and volunteering support
Support to access statutory services
How can I be referred?
You can access mental health support at your local GP practice. You can speak to a Mental Health Practitioner who can refer you to the Mental Health Community Connectors. Please note, we do not take self-referrals. You must be 16+ and be registered with a Rotherham GP.
