PURPOSE OF JOB
We are seeking a compassionate Family Coach to provide early intervention support to families accessing Sheffield Children’s Hospital services who are experiencing a range of complex needs. This role focuses on improving families’ health, social and emotional wellbeing by simplifying access to services, strengthening community connections, and empowering families to take control of what matters most to them.
You will:
• Work using a whole-family, person-centred approach, supporting families both within their local communities and alongside clinical teams at Sheffield Children’s Hospital.
• Taking a family-centred approach to create personalised support plans.
• Coordinating access to health and social interventions.
• Empowering individuals to share their views and take control of their wellbeing.
• Connecting families to local and city-wide services.
• Building connections and partnerships with health, social care, housing, and community organisations.
BACKGROUND
SOAR, Darnall Wellbeing, ZEST and Manor Castle Development Trust have come together to deliver a new and exciting pilot in partnership with Sheffield Children’s NHS Foundation Trust. The pilot project aims to close gaps in care and simplify access to services that address each family’s unique needs.
