Stroke Link Worker
In 2020 the Integrated Stroke Delivery Network recognised the importance of tackling the growing impact of stroke in England. It developed a new approach to give stroke survivors the chance of a better recovery and to prevent a re-hospitalisation.
Recommendations for stroke rehab services were made with enhancing connections between Community Stroke teams and the VCSE sector.
Funding was awarded throughout South Yorkshire and VAR employed their first Stroke Link Worker, Kay Hollis in September 2023. Kay takes referrals mainly from Rotherham Hospital Stroke Unit for patients within 12 months of having a stroke to support their recovery and facilitating a connection with their community to engage and fulfil their goals.
This enhances the well-being through a person led care plan, empowering individuals to live an independent life with a sense of purpose.
