We use cookies to identify the number of visitors to various parts of our website, this information is only processed in a way which does not identify anyone.

Rotherham Social Prescribing Service
If you agree to be referred to the Rotherham Social Prescribing Service, we will be provided with information from Health and Social Care teams and Mental Health Teams including:

your name, address, date of birth

health information, relevant to the social prescribing service
This information will be used to create your personalised support plan. To implement your support plan, we will share relevant information about you with Voluntary and Community services. We will do this with your explicit consent on being referred to the Social Prescribing Service. You have the right to withdraw your consent at any time, to request access to personal information we hold about you, to request that inaccurate or incomplete information is rectified and a right to restrict us from using your information in certain circumstances. If you would like to exercise any of these rights, please contact your VCS Advisor.
Your information will be kept for up to 8 years.
Your de-identified information will be shared with our funders, the NHS South Yorkshire Integrated Care Board and Sheffield Hallam University – Centre for Regional, Economic and Social Research (CRESR) – for monitoring and evaluation purposes. This will also be undertaken with your explicit consent, which you may withdraw at any time.
Marketing
We will not contact you for marketing purposes by email, phone or post unless you have given your prior consent.
You can change your marketing preferences at any time by contacting us.
How do we keep your information secure?
We are committed to ensuring that your information is secure. In order to prevent unauthorised access or disclosure, we have put in place suitable physical, electronic and managerial procedures to safeguard and secure the information we collect online. Our websites use Secure Sockets Layer (SSL) certificates to verify our identity to your browser and to encrypt any data you give us.
